What is Adjusted Body Weight?
Adjusted Body Weight (AdjBW) is used in clinical pharmacokinetics when a patient's actual weight significantly exceeds their ideal body weight (IBW). Many drugs distribute into lean body mass but not equally into excess adipose tissue, making actual body weight inaccurate for dosing.
The formula: AdjBW = IBW + 0.4 × (Actual Weight − IBW)
The 0.4 correction factor reflects that excess adipose tissue does contribute somewhat (about 40%) to drug distribution volume.
Ideal Body Weight Formulas
The Devine formula (1974) is most commonly used for IBW:
- Men: IBW = 50 kg + 2.3 kg per inch over 5 feet
- Women: IBW = 45.5 kg + 2.3 kg per inch over 5 feet
Adjusted body weight is typically used when actual weight exceeds IBW by more than 20–30%. When actual weight is at or below IBW, use actual weight for dosing.
Clinical Applications
AdjBW is used for dosing of drugs that don't readily distribute into fat tissue, including:
- Aminoglycoside antibiotics (gentamicin, tobramycin)
- Some chemotherapy agents
- Low molecular weight heparins
- Certain anesthetics
Always consult clinical guidelines and a pharmacist when making dosing decisions. Different drugs may use actual weight, IBW, or AdjBW depending on their pharmacokinetic properties.

Frequently Asked Questions
When should I use adjusted body weight vs ideal body weight?
Use AdjBW when a patient's actual weight is 20–30% or more over their IBW. Use IBW when actual weight is at or below IBW. Some drugs use actual body weight regardless of obesity status — always check drug-specific dosing guidelines.
What is the Devine formula for ideal body weight?
For men: IBW = 50 kg + 2.3 kg per inch above 5 feet. For women: IBW = 45.5 kg + 2.3 kg per inch above 5 feet. For example, a 5'10" man has IBW = 50 + 2.3×10 = 73 kg.
Is adjusted body weight only for medical use?
Primarily yes — AdjBW was developed for clinical drug dosing and pharmacokinetics. It's not a fitness metric. For fitness purposes, metrics like body fat percentage, lean body mass, or waist circumference are more relevant.
