What is Adjusted Body Weight?
Adjusted Body Weight (AdjBW) is used in clinical pharmacokinetics when a patient's actual weight significantly exceeds their ideal body weight (IBW). Many drugs distribute into lean body mass but not equally into excess adipose tissue, making actual body weight inaccurate for dosing.
The formula: AdjBW = IBW + 0.4 × (Actual Weight − IBW)
The 0.4 correction factor reflects that excess adipose tissue does contribute somewhat (about 40%) to drug distribution volume.
Ideal Body Weight Formulas
The Devine formula (1974) is most commonly used for IBW:
- Men: IBW = 50 kg + 2.3 kg per inch over 5 feet
- Women: IBW = 45.5 kg + 2.3 kg per inch over 5 feet
Adjusted body weight is typically used when actual weight exceeds IBW by more than 20–30%. When actual weight is at or below IBW, use actual weight for dosing.

AdjBW is used for dosing of drugs that don't readily distribute into fat tissue, including:
