What is Adjusted Body Weight?

Adjusted Body Weight (AdjBW) is used in clinical pharmacokinetics when a patient's actual weight significantly exceeds their ideal body weight (IBW). Many drugs distribute into lean body mass but not equally into excess adipose tissue, making actual body weight inaccurate for dosing.

The formula: AdjBW = IBW + 0.4 × (Actual Weight − IBW)

The 0.4 correction factor reflects that excess adipose tissue does contribute somewhat (about 40%) to drug distribution volume.
